"I will be your point of contact during the admission process. We do require a recent History & Physical from your Primary Care Physician, which I can help obtain with your authorization to release your Personal Health Information (PHI). I would also need insurance and financial information so our admission team can meet and approve admission. If needed I will reach out for further information. Once financially and clincially approved, I will reach out to you about an admission date. Day of admission it is required to have insurance cards, social security card, and drivers license if applicable so I can make copies. I will either have the resident or their representative fill out and sign our admission paperwork. If you have Power of Attorney paperwork, please bring the original or a copy.
